Accessing Your Conflict of Interest Disclosure Form

  1. Log in to the Access.Caltech web page: http://access.caltech.edu (Please note that a Virtual Private Network (VPN) connection is required for off-campus access.)
  2. Enter your Username and Password (this is the same as your IMSS/Kronos login credentials) and click the OK button.
  3. Select "Disclosure of Financial Interests and Commitments" under Administrative Services, and complete the form.

Disclosures are required annually or on an ad hoc basis as conflicts or potential conflicts arise.
